Suboxone treatment is conducted in a physician’s office setting and does not have restrictions such as daily clinic visits like that of other medication assisted treatment programs.

Proof of Income
- Most recent 1040 income tax return with W-2(s) and 1099
- Most recent two (2) pay stubs
- Earnings record from ssi.gov
- Proof of Social Security income, if applicable
- Verification letter if receiving food stamps
- Proof of alimony, child support, unemployment, pension, etc.
- If you receive no income and are being supported by relatives or friends, a notarized letter explaining those arrangements is required. The letter must be signed by person(s) lending assistance.
