Children’s Outpatient Care Coordination

Every child’s situation is unique. Often, complex cases require extensive hands-on coordination. In these instances, an AltaPointe care coordinator is assigned to assist families in obtaining needed services within our continuum of care and in the greater community.

Care coordinators may link families to resources available through the school system, the court system, and other organizations that provide assistance to people in need. From coordinating medical appointments to acquiring school uniforms, our staff members are experts at identifying needs and securing the support services necessary for success.
