AltaPointe Health provides residential care to adults with serious and persistent mental illness. Our residential facilities accommodate patients with a wide spectrum of needs based on their level of functioning and medical needs. AltaPointe provides housing, basic living skills, and a place where our patients feel safe and connected to their fellow residents and their community. Expected outcomes are the same for each adult group home: to prevent re-hospitalization and to maximize and maintain

Group Homes 

AltaPointe owns, operates, and staffs group homes designed to meet the unique needs of individuals requiring specialized care and support. These homes focus on rehabilitation and community reintegration and offer varying lengths of stay based on individual needs.

Residents receive nutritious meals, transportation to medical appointments, and opportunities for social outings. They can also access various supportive services, including peer support, psychiatric and medical care, and other therapeutic interventions.

Each day, residents participate in activities that build daily living, interpersonal, and coping skills. Our recreation department enhances the experience by organizing social events, fitness activities, fall prevention programs, and connections to community resources.

Family visits are welcomed and encouraged, providing vital support as residents work toward recovery and greater independence.

Foster Homes

Patients who require long-term housing and 24-hour care may be placed in one of the adult foster homes monitored by AltaPointe. Private contractors operate these residences and provide supervision, food, transportation, recreation and other services required for daily life. This program offers a less restrictive environment that focuses on the individual’s strengths in order to develop their basic living skills. These foster homes meet stringent AltaPointe standards and are closely monitored by AltaPointe personnel to ensure accountability and the best care available.
