Vendors who wish to do business with AltaPointe must follow the steps outlined below to be considered and approved. To expedite the process and ensure fair practices, we ask vendors to adhere to the procedures.
Public Hearing Notice: AltaPointe Health is applying to the Alabama Department of Transportation for a federal capital funding Award under Section 5310 of the Federal Transit Act.
Vendor Application Process
- Complete a Vendor Qualification Form
- Submit a cover letter briefly introducing your company, your products and/or services, as well as any business literature you would like to be reviewed.
- Complete a W-9 with the appropriate signature
- Mail your completed packet with all documents to :
