Navigating the proper course of treatment for a complicated mental illness can sometimes be overwhelming. These complex cases often require extensive hands-on coordination to maximize patient benefits. In these instances, case managers are assigned to assist patients in obtaining the services needed within the AltaPointe continuum of care and in the greater community.
In addition, our case managers provide outreach services connecting patients with a wide variety of state and local resources available through the government, the court system and other organizations that provide assistance to people in need. From help with coordinating medical appointments to basic life skills training, our staff members identify needs and assist with acquiring support services necessary for recovery.
