Vendor Forms and Information

AltaPointe Health works with local, regional and national vendors who can provide products and services that ensure our continued commitment to excellence in the care and service we provide to our patients, our community partners and our staff.

Vendors who wish to do business with AltaPointe must follow the steps outlined below to be considered and approved. To expedite the process and insure fair practices, we ask vendors to adhere to the procedures.

Vendor Application Process

  1. Complete a Vendor Qualification Form
  2. Submit a cover letter briefly introducing your company, your products and/or services, as well as any business literature you would like to be reviewed.
  3. Complete a W-9 with the appropriate signature
  4. Mail your completed packet with all documents to :
