Intellectual Disability Case Management

Our case managers have specific expertise in handling these complex situations and providing the intensive support required by these individuals.

What we do

Case management supports individuals with an intellectual disability by assisting them with integration into the community. Case managers assess individual need, develop an integrated care plan, advocate and link individuals to needed services, and regularly monitor each individual’s progress. To receive these services, individuals must meet the criteria as defined by the Alabama Department of Mental Health.

Developmental-Disabilities-ConsultationA case manager will connect patients with beneficial services within the AltaPointe system and in the community. Services are designed to link individuals that have challenges with adaptive functioning with necessary community support systems when they are unable to do so. The goal is to improve the individual’s quality of life in the least restrictive environment. These services meet clinical necessity, beneficial care and supportive service criteria.

Who we serve

Developmental-Disabilities-Case-Worker-Disabled-PatientWe provide services to children and adults in Mobile County who possess an IQ of 69 or below and who have been diagnosed with an adaptive functioning level of either profound, severe, moderate or mild. The diagnosis must be determined by the Inventory for Client and Agency Planning (ICAP) scale or other acceptable diagnostic tool. The individual receiving services must possess an Alabama Medicaid Waiver.
