Transitional-Age-Teens-GardeningTransitional Age Group Home

Our Transitional Age Group Home is a 10-bed, co-ed residential program that serves young adults, ages 17 to 22. Located in rural Mobile County, the setting offers space for outdoor activities such as gardening, sporting events and cookouts. In addition to standard indoor features, the home includes amenities such as computers, televisions, books, games, art and crafts.

Staffing – The home is staffed by a therapist, case manager, nurse, residential manager and behavioral specialist who provide individual, group and family therapy, psychiatric services, case management and 24-hour supervision.

Activities – Residents take part in school, work, field trips and activities designed to improve their levels of functioning. After patients graduate from the home to independent living, short-term counseling is provided as well as long-term case management to ensure a successful transition.

Adolescent Independency Program

Through a contract with the Alabama Department of Human Resources, AltaPointe provides services and support to young adults, ages 16 to 21, who have been released from DHR care, but need help establishing themselves in the community.

This home-like facility in rural Mobile County can accommodate 10 young men and women who need intensive residential services and are referred by DHR. It offers standard indoor amenities and plenty of space for outdoor activities.
