Navigating the proper course of treatment for a complicated mental illness can sometimes be overwhelming. These complex cases often require extensive hands-on coordination to maximize patient benefits. In these instances, case managers are assigned to assist patients in obtaining the services needed within the AltaPointe system and in the greater community.
In addition, our case managers provide outreach services connecting patients with a wide variety of national, state and local resources available through the government, the court system, charities and other organizations that provide assistance to people in need. From help with SNAP benefits (the Supplemental Nutrition Assistance Program known as “food stamps”) to coordinating medical appointment transportation or to basic life skills training, AltaPointe case managers identify patients’ needs and help them acquire the available support services necessary for them to recover.
