For patients with severe and persistent mental illness requiring a variety of services within AltaPointe and from various community resources
Every patient’s situation is unique. Often complex cases require extensive hands-on coordination to maximize patient benefits. In these instances, an AltaPointe case manager is assigned to assist patients in obtaining the services needed within our system and in the greater community. In addition, our case managers provide outreach services connecting patients with a wide variety of nationwide, state and local resources available through the government, the school system, the court system, charities and other organizations that provide assistance to people in need. From help with SNAP benefits (the Supplemental Nutrition Assistance Program known as food stamps) to coordinating medical appointment transportation, to acquiring school uniforms for our patients, the AltaPointe case manager identifies the patient’s needs and helps acquire the available support services necessary for the patient to thrive.
