AltaPointe Children’s Outpatient Case Management
Every child’s situation is unique. Often complex cases require extensive hands-on coordination. In these instances, an AltaPointe case manager is assigned to assist families in obtaining needed services within our continuum of care and in the greater community.
Case managers may link families to resources available through the school system, the court system and other organizations that provide assistance to people in need. From help with coordinating medical appointments to acquiring school uniforms, our staff members are experts at identifying needs and acquiring support services necessary for success.
