This program serves as a bridge between hospitalization and outpatient services. A multi-disciplinary team of AltaPointe professionals provides time-limited, intensive follow-up and support services designed to prevent decompensation and re-hospitalization. Through this intensive discharge planning and support, patients are better able to manage their symptoms and medications, and improve their lives through consistent therapy.
