Medication-Assisted Treatment (Suboxone)

Suboxone is used in medication-assisted treatment to help people discontinue their use of opiates – such as heroin and pain relievers such as morphine, hydrocodone and oxycodone. Suboxone treatment is conducted in a physician’s office setting and does not have restrictions such as daily clinic visits like that of other medication assisted treatment programs.

Required Information (one of the following):

Proof of Income

  • Most recent 1040 income tax return with W-2(s) and 1099
  • Most recent two (2) pay stubs
  • Earnings record from ssi.gov
  • Proof of Social Security income, if applicable
  • Verification letter if receiving food stamps
  • Proof of alimony, child support, unemployment, pension, etc.
  • If you receive no income and are being supported by relatives or friends, a notarized letter explaining those arrangements is required. The letter must be signed by person(s) lending assistance.

 Proof of No Income

  • Benefits verification letter from the Social Security office
